After a few days break. Thanks to one of our team for providing some documents that you may find interesting.

DWP "Guidance for Examining Medical Practitioners" . This is used by a company called Atos Origin who are a contractor for medical services for the DWP.

DWP "Disability Living Allowance EMP Report" this is the form filled in by a medic if you have a medical visit as part of your claim assessment process.
